Taking care of a loved one who needs medical attention can be overwhelming, especially when they’re coming home from a healthcare facility. The process of transitioning from hospital to home requires careful planning and attention to detail, to ensure the patient’s comfort, safety, and well-being.

At Aeratech, we have a 7-step process to make the transition as smooth and stress-free as possible for both the patient and their family. In this blog, we will take you through the 7 steps of our process, which are designed to provide comprehensive care, support, and education to the patient and their family.

  1. Assess the patient’s needs: We work closely with the case manager or referral source to learn more about the new patient’s needs and determine how we can best provide care.

  1. Establish a relationship with the family: We understand that trust and comfort are essential for effective care, so we meet with the family to establish a relationship that makes them feel secure and confident in our abilities.

  1. Conduct a home inspection: Before the patient comes home, we conduct a home inspection to ensure that there is adequate space and electrical sources to accommodate the necessary equipment.

  1. Provide comprehensive education: We provide comprehensive education to the patient, family, and caregivers on how to use the equipment and supplies, and we also provide reference materials for troubleshooting and FAQs.

  1. Provide support on discharge day: On the day of discharge for ventilator patients, we will be in the home to provide support, answer questions, and ensure that the patient is comfortable in their new environment.

  1. On-call support: Our licensed respiratory therapists are available after hours to answer questions or provide support for any mechanical difficulties.

  1. Easy resupply: We make resupply simple with the VerbalCare app. The family will receive notifications when it’s time to place an order, and they can easily request supplies, ask questions, and get updates on their shipment. The app also has a messaging feature for insurance changes, upcoming admissions, and sending necessary documents.

Our goal is to ensure a smooth and comfortable transition from the facility to the home environment so that the patient can thrive and enjoy their own space.
